HIV Screening Test

All new patients in Lambeth are offered a free HIV blood test. Please let us know if you would like this test and you can collect a test form to take to our local hospital.

NHS Organ Donor Registration

From 20 May 2020, organ donation in England will move to an ’opt out’ system. This means that all adults in England will be considered to have agreed to be an organ donor unless they have recorded a decision not to donate or are in an excluded group.

You still have a choice if you want to be an organ donor or not, for more information please go to www.organdonation.nhs.uk.

Summary Care Records

The Summary Care Record (SCR) is an electronic record of important patient information, created from GP medical records. This information can only be accessed by NHS healthcare staff involved in your direct medical care. The SCR is available wherever you need treatment, for example in an Accident and Emergency department.

As standard, the SCR holds:

• Your name, address, date of birth and NHS number
• Your current medication
• Your allergies and details of any previous adverse reactions to medications

You can also choose to share an enhanced record. This is particularly useful if you are elderly, or have complex or long term conditions .

The enhanced SCR holds:

• Significant medical history (past and present)
• Reason for medications
• Anticipatory care information ( information about the management of long term conditions)
• End of life care information
• Immunisations

If you opt out of the SCR, NHS healthcare staff caring for you may not be aware of your current medications, your allergies or adverse reactions to medications you have had. This may impact the safety of treatment you receive in an emergency.

Electronic Prescription Service

The Electronic Prescription Service (EPS) is a system that allows the surgery to send prescriptions electronically to a pharmacy of your choice. This makes the process of getting your medication easier, allowing you to collect directly from the pharmacy.

Carer Information

A carer is anyone who cares, unpaid, for a family member or friend who due to illness, disability, a mental health problem or an addiction, cannot cope without their support.
